Description

This Lupus Anticoagulant test uses Russell's viper venom to directly activate factors V and X, bypassing the tissue factor pathway and the contact and antihemophilic factor of the intrinsic pathway. The ratio of clotting times with excess phospholipid to that without excess phospholipid is used to determine the presence of lupus inhibitor in the plasma. This test will be added by the laboratory reflexively if Hexagonal Phospholipid assay is negative with prolonged PTT above 40 seconds.

For patients on Xa inhibitors (Apixaban, Rivaroxaban, Edoxaban), order Hexagonal Phospholipid assay (HEXPI) instead.

Interferences and Limitations

Patients on direct oral anticoagulants (Bivalirudin, Dabigatran, Rivaroxaban, Apixaban, Edoxaban) have prolonged clotting time with the DRVVT assay and may give false positive results depending on the drug concentration in the plasma.
