UW Travel Services Website: Information on policy/reimbursement and fiscal compliance in accordance with University, state, and federal requirements.
Travel Forms:

Travel Approval Form (pdf): required before traveling. Division head and chair’s approvals are required; email approvals are sufficient.

CTA Form (pdf) (if applicable): required for pre-travel arrangements by the Department (flight, hotel).

Travel Reimbursement Form (pdf): required post-travel to keep track of expenses for UW employees.

Non-UW Employee Travel Reimbursement Form (pdf) (if applicable): required post-travel for non-UW employees (no need to fill & submit the DLMP travel reimbursement form; this will replace that for non-UW travelers only).

EH&S
Environmental Health & Safety facilitates institutional committees with responsibilities for research safety and compliance, occupational health and safety, and environmental protection at the University. They have various resources and information for the setup, maintenance, and decommissioning of laboratory environments.
Setting Up a Lab
Laboratory Safety Manual: Staff in UW laboratories using hazardous chemicals must have access to a copy of the manual and the supplemental laboratory-specific information. The manual is updated annually.

Biosafety Manual: All laboratories and facilities using biohazards should have an electronic or paper copy of the current Biosafety Manual available. This manual is updated annually.
- UW Biosafety Manual (pdf): Guidelines for handling possible hazards associated with the use of biohazardous agents and recombinant or synthetic DNA/RNA (recDNA) molecules.
Lab Safety Dashboard: Visit the Laboratory Safety Dashboard page to access the appropriate portal to the dashboard. Inspection reports and metrics are available to view based on your access level.

PI Dashboard: Additional tools accessed on the PI portal of the dashboard include a self-inspection tool, a staff list, and lab equipment records.
The dashboard also shows the safety rating calculated from the inspection results.

LAB SAFETY OFFICE HOURS
- Lab Safety is now offering a monthly virtual office hour on the last Tuesday of every month 3pm - 4pm via Zoom (UW NetID required).
- Bring your questions about laboratory safety documents, practices or concerns to the virtual Lab Safety Office Hour.
