Description

Useful For: Karius™ is a non-invasive liquid biopsy test for rapid detection of over 1000 pathogens. Refer to linked website below for a list of pathogens currently detected by the Karius™ assay.

Ordering Requirements: Indications for this test are rare. Approval from Microbiology/Virology faculty (via Lab Medicine Resident) is required prior to sample collection. The Laboratory Formulary Committee established a limit of 20 total mNGS sendout assays per year.

PSPS:

  • Epic orders will be placed as LABUND (Laboratory Undefined).
  • If the LABUND order does not indicate previous approval, contact the Laboratory Medicine Resident (LMR) on-call to obtain approval prior to sample collection.
  • Once approval has been obtained and communicated to SPS, the sample may be collected and processed.
  • Questions regarding sample collection or processing should be relayed to SPS or to the resident. Do not contact Microbiology Lab with questions.

Processing

PEARL PPT: Centrifuge samples within 6 hours of collection. If only one pearl top was collected and sufficient sample is available, make an extra aliquot for test UW434 into a sterile plastic vial using sterile technique.

K2-EDTA: Centrifuge sample and aliquot 0.7 mL plasma each into two sterile plastic vials using sterile technique (contact Micro lab for assistance). Do not disturb the buffy coat when transferring.

If there is insufficient plasma available, prioritize the sendout test.

  • RKARUS: Store samples in the ambient Sendouts rack.
  • UW434: Forward to Molecular Micro for sample storage (-80°C mNGS box).

If shipment is delayed: Samples >4 days old should be frozen at -20°C. (For PPT, freeze centrifuged specimen in original collection tube without aliquotting.)

Sendouts:

  • Contact CAST for assistance with online ordering and requisition printing. Karius orders are considered urgent and should be expedited as much as possible.
    • CAST: The name and contact information of the ordering provider should be included in the order for direct result alerts.
  • Ship the sample the same day as collection (if possible) at ambient temperature using the Karius™ kit. Wrap the sample with the provided gel pack to maintain temperature control. Do not use the enclosed shipping label for UPS pickup. Generate a separate FedEx label and select the "First Overnight" option.
  • Samples may be shipped on Friday for Saturday delivery.
  • Do not ship frozen samples in the Karius™ kit. Create a separate FedEx shipment label using the online FedEx account and ship on dry ice.

Stability: Ambient (preferred): 96 hours; Frozen: 6 months (spun, frozen PEARL PPT tube is acceptable).
