Please allow up to three weeks for your verification to be completed. To expedite the process, make sure a signed release form is attached to your request.

 

Verification Fee

A $125 processing fee applies per verification request and is paid by the requesting organization. If residency and fellowship are verified together, only one fee is charged. Processing training verification may begin once the requester confirms the check is mailed.

Fee Exemptions

No fee is charged if:

• Trainee has been out of the program less than two (2) years
• Program is non-ACGME accredited (e.g., Clinical Chemistry, Microbiology)
• Program is for Post-Sophomore Fellowship Program
• Request comes from an affiliated institution, including:
  • University of Washington Medical Center 
  • Harborview Medical Center
  • Seattle Children’s Hospital
  • Fred Hutchinson Cancer Center
  • King County Medical Examiner’s Office
  • VA Puget Sound
  • Bloodworks Northwest
  • Requests from: American Board of Pathology, American Board of Dermatology, State medical boards, USMLE, Letters of reference completed by faculty
