Description

Test Description: HeartCare is used for surveillance of heart transplant recipients for acute rejection in conjunction with standard clinical assessments. HeartCare testing may be ordered for heart transplant recipients who are 2 weeks (≥14 days) post‐transplant (the AlloMap component of HeartCare may begin day 55 post‐transplant). HeartCare testing includes AlloMap (Sendout) [RALLMP] and AlloSure-Heart (Sendout) [RALLSR].

Ordering Restrictions:

  • Laboratory Medicine Resident (LMR) approval is required for inpatient orders.
  • This test is available to UW Medicine patients only.

Collection

Collection Restrictions: Due to the time-sensitive processing required, orders must be pre-scheduled with UWMC Montlake Outpatient Blood Draw. AlloMap® samples must be processed within 3 hours of draw time. This test cannot be drawn at other collection sites.

Collect:

  • 20 mL whole blood in 2x10 mL Streck Cell-Free DNA BCT® tubes (mottled black and tan).
  • 10 mL whole blood in CPT™ Cell Preparation Tube (mottled black and blue).
  • For AlloSure-Heart (Sendout) [RALLSR], lab staff may use Streck tubes on hand instead of those associated with a CareDx kit. However, a CareDx kit is required for the AlloMap (Sendout) [RALLMP] portion of the HeartCare test.

Approval Required

Laboratory Medicine Resident (LMR) approval is required for inpatient orders.

Handling Instructions

Outside Laboratories: This test requires special processing and is offered to UW Medicine patients only. Outside laboratories should arrange for testing directly with CareDx.

Processing

Phlebotomy:

  • Print the CareDx order form provided by the Transplant Clinic. Receive the EPIC order in Sunquest and print labels.
  • Draw blood using the samples included in the HeartCare kit (2 Streck tubes for AlloSure® and 1 CPT™ tube for AlloMap®).
  • Label the samples with the Sunquest labels.
  • Affix Sunquest-generated accession labels to both the CareDx order form and the EPIC order.
  • Record the collection date, collection time, and phlebotomist initials on the CareDx order form.
  • Package the AlloMap® sample (CPT™ mottled black/blue tube) and CareDx Order Form in the AlloMap® kit (do not seal) and place it in the designated courier pickup bin for Bloodworks Northwest.
    • Courier pickup will occur every two hours, with the final pickup at 3 p.m.
    • AlloMap® samples will be processed and sent out by Bloodworks Northwest Platelet Immunology Laboratory.
  • Send the AlloSure® samples (Streck mottled black/tan tubes), AlloSure® kit, and EPIC requisition (along with any other samples from the order) to Specimen Processing Services.

SPS: Store whole blood at room temperature.

  • If the AlloMap® sample (CPT™ mottled black/blue tube) is received in error, alert Phlebotomy by calling 206-598-6153 and immediately tube the sample (along with AlloMap kit and CareDx form) back to Tube Stop #430.

Sendouts (AlloSure-Heart®):

  • Label the tubes and the requisition with the provided barcode labels (ensuring that the expiration date is visible). Leave the barcode labels blank (additional demographic information is not required as long as the tubes are labeled with LIS-generated labels).
  • Affix a Sunquest accession label to the EPIC requisition or the CareDx HeartCare form.
    • Accession labels are required to correlate the AlloSure and AlloMap orders that will arrive at CareDx separately.
  • Package samples and ship at room temperature using the provided kit. Samples must be properly packaged with the ambient gel pack or orders will be cancelled by CareDx.
  • Okay to ship Friday for Saturday delivery.

AlloSure-Heart® Stability: Ambient (Required): 7 days; Refrigerated: Not Acceptable; Frozen: Not Acceptable.

AlloMap® Stability: Samples must be processed by Bloodworks Northwest and placed in freezer within 3 hours of collection.
